[abstract] objective to evaluate the effect of pullthrough combined with double stapling technique for sphincter preservation operation in low rectal carcinoma. methods pullthrough combined with double stapling technique for sphincter preservation operation was performed on 25 patients with low rectal carcinoma form may 2004 to december 2007. the clinical data were analyzed retrospectively. results fecal continence was preserved successfully in all patients. there was no operative death. only one case got anastomosis leak and 2 cases had incision infection after operation. patients were followedup for 13 years, and two patients had local recurrences. conclusion pullthrough combined with double stapling technique for sphincter preservation operation is simple, safe and feasible for low rectal carcinoma, and enhance the quality of life.

我国直肠癌的特点是低位即距齿状线5 cm以内、腹膜返折下的癌多达70%～75%[1]。WWW.133229.cOM直肠癌是人类最常见的恶性肿瘤之一，近年来发病率呈上升趋势。随着人们对生活质量要求越来越高，需做永久性人工的低位直肠癌患者越来越渴望保留。吻合器的临床应用使保肛指征更加广泛。我院 2004年5月至2007年12月采用经拖出双吻合器保肛手术治疗低位直肠癌患者25例，取得满意效果。现报告如下。
1 临床资料
1.1 一般资料 本组25例中,男18例，女7例；年龄32～73岁，平均52.5岁。肿瘤距齿状线3～5 cm。所有患者术前均获得病理确诊。组织学分型:高分化腺癌12例，中分化腺癌11例，低分化腺癌2例。术前进行辅助检查排除远处转移，并行直肠腔内超声、ct、mr检查，明确肿瘤未侵犯肛管及未穿出肌层，肿瘤直径<3 cm。
1.2 手术方法 膀胱截石位，取下腹正中切口。常规切开腹壁各层后，游离直肠上动静脉，动脉内推注5fu 500 mg后，切断并结扎直肠上血管。按无瘤操作技术，结扎近端肠管。行直肠系膜全切除(tme)手术，手术从左侧游离乙状结肠，解剖出肠系膜下动静脉，靠近根部切断结扎并完成肠系膜淋巴结清扫 。直视下用电刀在前盆筋膜壁层与脏层间锐性分离直至肛提肌平面，保证直肠及其全部脂肪系膜组织被完整切除，下端至肛提肌平面，?；は的ぴ嗖悴黄扑?，以防癌细胞播散、种植和残留。在处理直肠侧韧带时靠近盆壁锐性分离，注意保留自主神经丛。完成肿瘤肠段游离后，距肿瘤10 cm切断乙状结肠。扩张至可容纳4指后，将切断的乙状结肠、直肠(包括肿瘤)从直肠肠腔内套叠拖拉外翻至外，显露肿瘤及齿状线。使用稀释的碘伏溶液反复冲洗拖出的直肠远端，无张力下距肿瘤远端2 cm处用闭合器关闭远端直肠，切除乙状结肠、直肠及其肿瘤，移除标本。术中送直肠下切缘进行快速冰冻病理检查，证实无肿瘤残留时，回纳复位直肠的残端至盆腔。用管状吻合器在盆腔内吻合乙状结肠与直肠,吻合完毕后，用肠钳夹闭吻合口上方的乙状结肠，盆腔内注入冲洗的生理盐水并浸过吻合口。经注入气体，观察吻合口处有无气泡冒出，如有气泡，则应加针缝合。于骶前放置乳胶引流管，自旁戳孔引出并妥善固定。
1.3 结果 本组25例吻合过程顺利，全组无手术死亡病例。术后发生切口感染2例，吻合口漏1例。经过保守治疗后痊愈。术后1～3个月，患者排便能力正常(大便2～3次/d，成形便)，随访1～3年，有2例局部复发，再次行miles手术。
2 讨 论
2.1 直肠癌手术切除的理论依据 低位直肠癌的定义是指距齿状线5 cm以内的直肠恶性肿瘤。低位直肠癌保肛的前提是保证肿瘤远端直肠切缘无癌组织残留。低位直肠癌的淋巴引流主要向上，极少向下逆行扩散，仅在晚期、高度恶性、向上引流的淋巴管被癌栓堵塞后才会向下逆行扩散，且其扩散的范围是极其有限的,很少超过2 cm。逆行扩散>2 cm的病例，均属分化不良的高度恶性病变[2] 。ruile等[3]报道miles术与低位保肛术的局部复发率分别为21%与28% 。直肠癌的病理、生物学行为研究表明，切除远端肠管的平面距肿瘤下缘约2 cm，则不会构成肿瘤复发的因素[2]。大宗病例研究已证实，切除远侧肠管2 cm与切除5 cm者的术后局部复发率、生存率等差异均无统计学意义。一般情况下，切除长度只要大于2 cm即已满足根治要求[3,4]。对于恶性程度不高的直肠癌，远端直肠切除不少于2 cm是安全可靠的。因为直肠是紧贴于骶骨凹上形成弯曲，术中游离直肠达盆底后，直肠可延伸3～5 cm，从而使部分术前看似不能保肛者得以保留。但是否保肛主要是依据患者的全身情况、肿瘤的分化程度、浸润转移的范围以及距齿状线的距离而个体化对待。
2.2 低位直肠癌经拖出吻合手术操作要点与并发症预防 ① 严格遵守tme手术操作原则，直视下在骶前间隙、盆筋膜脏层与壁层间行锐性分离；保持盆筋膜脏层与壁层的完整无损，分离时切勿进到直肠系膜内；直肠分离要超出尾骨尖，必须切断直肠骶骨筋膜，切断肛尾韧带。必要时扩大直肠远端分离范围，使直肠充分游离，正确估计肿瘤下缘与肛提肌的距离。② 保护输尿管及盆部重要脉管。由于晚期肿瘤很容易粘连输尿管及盆部重要脉管，故术中我们常规暴露输尿管及髂外动脉，以免意外损伤。③ 吻合时应选择偏大的吻合器，防止吻合口狭窄。④ 充分扩肛至可容纳4指，闭合器夹在肿瘤下缘下2 cm处，紧贴闭合器切断肠管，防止残缘过长、翻卷入钉座的肠壁过厚、击发不全出现吻合口漏。⑤ 清除结肠残端肠壁外0.5～1 cm内的脂肪组织，避免吻合口周围组织嵌入过多，影响吻合口愈合或术后吻合口狭窄。⑥ 吻合器击发前一定要检查无周围组织夹入，肠管不扭曲，力争做到一次彻底，击发后停留约10 s，压迫残缘以防出血。⑦ 吻合完毕退出时要缓慢、轻柔、切忌暴力，退出后检查切缘圈是否完整，如不完整应及时修补吻合口。本组吻合口漏1例，经过保守治疗后痊愈。
2.3 直肠癌保肛手术的适应证 实施直肠癌双吻合器技术保肛手术时，首先强调癌肿的根治性，同时亦要求保存功能或恢复到自然状态。应掌握以下手术适应证[5]：① 高、中分化腺癌，癌肿距肛缘5 cm以下(或距肛管直肠环2 cm以上)，癌肿未侵犯肠壁浆肌层，或虽侵犯浆肌层而癌肿周围能切除干净者，肿瘤直径<肛管直径。② 分化差的癌肿如低分化、未分化、黏液腺癌等。侵犯肠壁周径1/3以下，距肛缘7 cm以上，肠管周围可切除干净者。 ③ 无前列腺、阴道、膀胱浸润者。④ 癌肿虽有腹腔内其他部位转移，而局部符合上述条件，行姑息性原发病灶切除时，亦可行保肛手术。
2.4 经拖出双吻合器保肛术治疗低位直肠癌优点 ①切断的乙状结肠、直肠(包括肿瘤)从直肠肠腔内套叠拖拉外翻至外。有助于确定肿瘤的远切端以确?；竦贸浞值闹琢銮性?。经肛管拖出后，克服了盆腔狭小，手术操作困难的缺点，可使病变肠段黏膜在直视下得以彻底清洗从而避免肿瘤细胞的污染和播散。②吻合器应用于低位直肠癌患者或肥胖、骨盆狭小的患者，缩短了手术时间，提高了低位直肠癌做前切除的机会。③采用双吻合器吻合，吻合口漏发生率也低于经手工吻合者。④因为吻合在盆腔内进行，大多可避免松解结肠脾曲，简化了手术操作，减少了手术创伤。⑤保留括约肌，保留了的排便功能，避免了低位直肠癌永久性人造给患者带来的痛苦 。在根治的前提下，患者的生存质量有了很大的提高。

（1 ）代为领取有关设立该贸易公司的表格及相关资料，填写该等表格，起草、签署和提交有关的申请报告、文件及政府部门要求提交的所有其他相关文件，向有关政府部门提交该等表格、文件、资料和证件；
（2 ）代为办理外商投资项目名称预先核准、报批、工商登记及其他相关登记、批准等手续，并代为领取有关的批复、批准证书和营业执照等；
（3）为设立该贸易公司之目的，代为办理和处理其他任何必要的手续和事宜。
为上述（1 ）至（4 ）之目的，受委托人有权在必要时对其认为胜任的第三方予以转委托。
本委托书自本人签字之时起生效，自委托事项全部完成之时终止。
